He leaves the castle, yes, but that was not the end of the book. That was like 3/4 into the book. After the duel between Dumbledore and Voldemort at the ministry, Harry and DD have a long conversation in his office (he's reinstated as Headmaster) about the prophecy and several other things. After their conversation though, we don't see DD until chapter 3 of HBP.

At the end of OotP Dumbledore returns as Headmaster of Hogwarts and everyone's faith in him is restored. Most people no longer think he's a liar.

Snape kills Dumbledore (though there is much speculation as to the exact details of Dumbledore's death.)

Yes. He explains the prophecy to Harry, which he kept in 5 years. I remember Harry destroying Dumbledore's things lol
