How he escaped Azkaban: Dementors can't see. They're blind. Crouch Jr's mum was dying. Crouch Jr was dying. That's all the Dementors knew. The switched spots, and Crouch's dad disguised Crouch Jr as his mum, in case a guard was watching. They walked away, 'cause all the Dementors 'saw' was a sick person and a healthy person leaving, the same as what came in.
He needed to be controlled, I assume, so that he didn't let their secret out. Imagine what would have happened had Crouch Jr gone outside as himself, or visible. His dad most likely would have been arrested for aiding in a prisoners escape, and he would have been tossed back into prison... So his dad Imperio'd him, and made sure he wouldn't be seen.

The dementors can't see, but they can sense soulos. They sensed two souls entering Azkaban and two souls leaving. They didn't care which, as it never occurred to anyone that someone would actually be willing to switch places.
Now, Barty Jrs. mom was dying and as her dying wish she wanted to switch places with her son so he could have freedom and she would spend her remaining days in Azkaban. Barty Sr. took his wife with him to Azkaban... once there, they switched places. His mother made sure she had polyjuice potion and she was very careful about taking some until the very end so no person would see that it wasn't Barty Jr
Meanwhile, on the outside, Barty Jr had to remain hidden at all times, so no one would suspect anything. His father kept him under the Imperius Curse so he wouldn't try to leave or worse. Winky was left with him to care for him.
