I kinda figured it was whatever form Voldie had had before Wormtail dropped him in the cauldron in the Goblet of Fire.
In a basic matter, it was Voldie's soul (in my opinion) which was so rotten and pathetic that in its truest form, it was this sort of baby sized disgusting squirmy thing.
Ya know, tha kindof thing that if you were to see it in the street you'd wanna kick it... |

yep is was the the last horucrux of he-who-must-not-be-name, when he wanted to kill harry as a baby, but since it backfired on his ass he's soul went inside harry without him knowing it...lets say (like in the book) it latched on the nearest living thing.
